Making your complaint
Appointment Complaint:
To make an Appointment complaint, please complete the appointment permission form and monitoring form and return it to us.
Conduct Complaint:
If you wish to make a conduct complaint you should complete the conduct complaint form and monitoring form and return it to us.
All complaints:
You should include the following:
- your name and preferred contact details;
- details of your complaint; and
- any information that you feel would assist the Ombudsman in investigating your complaint.
Complaints should be set out using the space on the forms provided.
If not already authorised we will ask for your permission to disclose your complaint before proceeding further. In most cases it will be impractical to proceed with a complaint if permission is withheld.

We will acknowledge your complaint within five working days of receipt and will also let you know how we will investigate your complaint and give you regular updates on our progress. At the end of our investigation we will send you a letter or report explaining our final decision.
